Mr Fowler has practiced as a Consultant Orthopaedic and Spinal Surgeon since 1992. His NHS practice is at Southampton University NHS Healthcare Trust. He has been instrumental in the development of the combined Spinal Surgical department in Southampton which has brought together a team of Orthopaedic Spinal Surgeons and Neurosurgical Spinal Surgeons. He is committed to further develop these associations in his work for the national organistions of BASS (British Association of Spinal Surgeons) and UKSSB (UK Spinal Surgery Board). Mr Fowler is dedicated to training of new Orthopaedic and Neurosurgical trainees as Spinal Surgeons.

He has developed minimally invasive techniques, which allow reduced length of hospitalisation and greatly benefit recovery and return to work. Spinal surgery is a branch of surgery that has developed incredibly in the last 20 years. Much of this development has been possible due to the introduction of detailed anatomical imaging techniques together with the technical advances of implant design and manufacture. Just as hip replacement surgery was pioneered in the 1960s and matured to the highly reliable procedure of the 1990s, we are seeing a sophisticated speciality develop in spinal surgery. The challenges are greater, the surgery more complex and patient improvement can be even more dramatic. Procedures, that a few years ago were massive interventions with extensive wounds and muscle damage, are now possible with minor incisions, less discomfort and a rapid return to activity for the patient.

Mr Fowler, the teams at University Hospital Southampton, Nuffield Hospital Hampshire, and at the Sarum Road Hospital have been using the TOPS System since December 2012.
